Person Centered Planning
Haufe Person Centered Planning Services is a company established to offer planning services to individuals diagnosed with developmental disabilities, seniors, or others and their families in identifying a safe and secure future. Sarah Haufe is the owner, president and lead facilitator of the company; she holds a BA in psychology and sociology as well as a master’s in conflict resolution.
Sarah has worked front line social services for over a decade as a case manager, resource facilitator and planner for young adults diagnosed with developmental disabilities. More recently she has begun teaching the next generation of social service workers aiding them in developing the skill sets required to successfully work in the field of social services and decided to launch Haufe Person Centered Planning Services.
Services Offered
Person Centered Planning – a facilitated planning process designed to identify strengths, challenges and barriers to a safe and secure future and then establish an action plan with appropriate services and supports to successfully accomplish the goals.
Form Completion – assistance for individuals and families completing forms for access to services, supports and resources such as ACSD, SSAH, ODSP and Passports.
